Same Cage Boarding Consent

I authorize the above said pets to be boarded together in the same cage during their stay at the 4 Paws Veterinary Care. I understand the risk of conflict, and authorize treatment at owner expense if required.

I understand the pets boarding may be separated at the discretion of 4 Paws Veterinary Care staff for safety purposes. If separation is required the discounted rate would revert to the regular boarding rate for the remainder of the stay.
